Membership Requirements

1. Documentation Papers

• For Corporations

- SEC Registration Certificate
- Articles of Incorporation
- By-Laws
- BIR Registration Certificate
- Taxpayer Identification Number (TIN)
- Current Business and Mayor’s Permits from the City or Municipal Government where entity is registered
- BIR ruling that income from royalty is considered as active rather than passive ( if any )
- Board Resolution authorizing Company Representative

• For Partnership

- SEC Registration Certificate
- Articles of Co-partnership / Partnership
- Taxpayer Identification Number (TIN)
- Current Business and Mayor’s Permits from the City or Municipal Government where entity is registered
- Board Resolution authorizing Company Representative

• For Sole Proprietorship

- Certificate of Registration with DTI
- Taxpayers Identification Number (TIN)
- Current Business and Mayor’s Permits from the City or Municipal Government where entity is registered
- Special Power of Attorney (SPA) authorizing Company Representative

3. FILSCAP Deed of Assignment (please print 5 sets and sign on each page for all sets)

4. Updated List of songwriters and assigned works/songs (with tapes/CDs)

5. Any Government Issued ID of the Company Representative

6. Payment of P 1,000.00 entrance fee and P700.00 annual membership fee (to be paid after admission as a member).
